Chalazion at The London Eye Clinic

A chalazion (also known as a meibomian cyst) is a cyst in the eyelid. A common condition, it often but not always occurs due to inflammation around the opening of the oil glands at the base of the eyelashes.

  • If the lump is large enough, it can press on the eyeball causing blurred vision
  • A Chalazion can interfere with tear production, leading to an irritable or watery eye
  • The lump is often painless, though it can cause mild irritation or tenderness, especially when it first appears.

What is Chalazion?

A chalazion (also known as a meibomian cyst) is a cyst in the eyelid. A common condition, it often but not always occurs due to inflammation around the opening of the oil glands at the base of the eyelashes. The inflammation (called blepharitis) is caused by sensitivity to a common bacteria found on the skin. When the opening of the small oil glands around the lashes become blocked by inflammation, a small tender swelling occurs in the eyelid, known as a chalazion. The chalazion will often vary in size over a few weeks and can discharge spontaneously or disappear after hot compresses and lid cleaning. A small proportion of the chalazia will remain for weeks or months in the lid, as a non-tender lump; this can be safely left to get better by itself. Sometimes the chalazion may cause a spreading infection along the surface of your lid, which might need a course of antibiotics by mouth, but this is rare.

There are some measures you can take which may help resolution. These include:

Warm compress: Boil some water and let it cool a little or use water from the hot tap. Soak cotton wool or a clean flannel in the warm (but not boiling) water, squeeze it out and gently press onto your closed eyelids for two to three minutes at a time. Alternatively, you can use heated eye masks

Massage: Starting at the skin crease, gently massage the upper lid downwards to the lashes. The lower lid should be massaged upwards towards the lashes. This may release the contents of the cyst, which means further treatment will not be necessary.

If the chalazion does not respond to the first stage of the treatment, it can be drained, by a procedure carried out in outpatients under local anaesthesia referred to as an “Incision & Curettage” (I&C), although it can also be left alone.

When to Seek Medical Help?

Signs of Infection: Look for increased redness, significant swelling, pus, or fever.
Severe Pain: While usually painless, severe or recurring pain can signal an infection.
Persistent Symptoms: If symptoms persist or worsen after a few days.

Treatment

Information on the procedure

The procedure which is referred to as and Incision & Curettage (I&C), is carried out as an outpatient procedure in clinic and can often be offered on the same day as the consultation.

After an injection of local anaesthetic, the lid is everted, and a small incision is made in the under surface of the cyst and the contents expressed (scraped). Occasionally the procedure needs to be carried out through the front skin of the eyelid or the eyelid skin can break if already affected by the inflammation from the cyst. The eye is then padded for a few hours and you are provided with a course of antibiotic drops or ointment.

  • Commonly, the lid can be swollen or bruised for a few days after the procedure
  • You can wash your face and eye and shower as normal
  • You may experience a foreign body sensation in the eye, as though there is an eyelash or something under the eyelid
  • You can experience some mild intermittent discharge, which may be bloodstained. If present, these symptoms usually subside  within a few days
  • You should not wear a contact lens in the treated eye for at least one week after an I&C
  • You may feel a small residue of the swelling, which may take a little longer to settle

Even if the treatment has not worked, we would wait at least six weeks before considering a repeat incision and curettage as many can continue to settle during this period.

This is generally a safe procedure, but no intervention is risk free. The risks of I&C are small but include failure, recurrence, corneal abrasion, infection, bleeding, lash loss (very rare), and breakage through the skin overlying the cyst if already thinned, crusted and scabbed due to partial spontaneous discharge. This usually heals very well.

About Ectropion

How does Ectropion happen?

The problem is usually caused by ageing. As you get older, your skin and soft tissues of your eyelids slacken. Similar to a tennis net which is not tight at the ends and will sag in one direction or another, the lid margin will turn outward. Infrequently, the problem is caused by scar tissue on the skin of the eyelid pulling the lid margin outwards and away from the eyeball. Ectropion can cause irritation of your eye and eyelid, leading to symptoms such as an unsightly appearance, a watery eye, discomfort, foreign body sensation, grittiness, discharge, dryness or redness.

What are the benefits of surgery?

Patients opt to undergo surgery either because the eye is watering, although ectropion may not be the only reason for a watery eye or concerns about the appearance or discomfort

Are there any alternatives to surgery?

There are no permanent alternative treatments, but the symptoms can be managed to an extent. Artificial tears and ointments can be used and may relieve some of the discomfort.

What will happen if I decide not to have the operation?

With ectropion the symptoms which you are presenting with will persist. Whist these symptoms may not be desirable, they are unlikely to cause any permanent harm to the eye.

Entropion at The London Eye Clinic

Entropion is where your eyelid turns in and causes your eyelashes to rub against the surface of the day, causing irritation and foreign body sensation.

Entropion can usually be treated by an operation. About Entropion

How does entropion happen?

The problem is usually caused by ageing. As you get older, your skin and soft tissues of your eyelids slacken. Infrequently,  the problem is caused by scar tissue on the inner linings of the eye pulling the lid margin inwards. The lower eyelid is commonly affected. Entropion irritation of your eye and eyelid, leading to symptoms such as an unsightly appearance, a watery eye, discomfort, foreign body sensation, grittiness, dryness or redness.

What are the benefits of surgery?

The main reason for undergoing surgery is to achieve comfort. Occasionally patients do not experience significant discomfort, but the in turning lashes would increase the risk of post-operative infection, if they are to have operations such as cataract surgery and therefore an operation to correct the entropion before their eye surgery would be recommended. Your eyelid should be in a better position and your symptoms should improve.

Are there any alternatives to surgery?

There are no permanent alternative treatments, but the symptoms can be managed to an extent. Artificial tears and ointments can be used and may relieve some of the discomfort. Occasionally a special soft contact lens (bandage contact lens) can be applied as a temporary measure to protect your eye until you have an operation.

Other conservative measures for entropion can include:

  • Using tape to prevent your eyelashes from rubbing against your eye. The tape can irritate your skin if it is used for more than a few weeks.
  • Injecting Botox into the muscle that turns your eyelid inwards (orbicularis muscle). This may work by paralysing the muscle and can relieve your symptoms, but even if effective the effect will wear off and would need to be repeated.
  • Using stitches (called everting sutures) to prevent your eyelid from turning in. This is a very quick and effective, but may not stand the test of time and need to be repeated.

What will happen if I decide not to have the operation?

Your symptoms may get gradually worse with time. Sometimes ulcers can develop on the surface of your eye, leading to infection, scarring and reduced vision.

I just don’t know how to thank you for my improved eye sight. Everything is clearer, brighter and the colours are wonderful! The right eye could not have been more comfortable; in the left eye I had some very slight discomfort. What causes Ptosis?

There are many causes of ptosis such as neurological or muscle problems, but by far the commonest cause of ptosis is age related degeneration, weakness, stretch, dehiscence or disinsertion of the muscle, from the lid cartilage, which holds the eyelid in position.

What is the treatment of Ptosis?

This depends on the cause, but as the commonest cause of ptosis relates to problems with the muscle of the eyelid, the treatment involves surgical tightening (repair or shortening) of the eyelid muscle. It is different to blepharoplasty, which involves removal of excess skin or fat from the eyelid.

What results can I expect after Ptosis surgery?

Ptosis surgery is designed to lift the eyelid into a more natural position, improving both appearance and vision. Most patients notice an immediate improvement in how open and balanced their eyes look, with results that continue to refine as healing progresses. While every case is unique, the aim is always to achieve a natural-looking outcome that restores confidence and comfort.

Information on the procedure

Ptosis surgery is typically carried out under local anaesthesia, sometimes with sedation, with the patient awake. An incision is made in the crease of the eyelid; the muscle is identified and tightened. The lid height and contour is then adjusted with the patient’s cooperation by opening and closing the eyes, until the optimal eyelid height and contour is achieved. The surgery can be carried out on both lids simultaneously, if needed.

Your Procedure

An incision is made in the eyelid crease, the muscle is tightened, and the lid height and contour are adjusted with patient cooperation until optimal results are achieved.

Recovery & Follow-up

After surgery, expect temporary bruising, swelling, mild discomfort, and stitches (removed within a week), with treatment including painkillers, eye drops, and antibiotic ointment.
